HOW TO SEND AN APOLLOGRAM?
OPTION #1: HAND-WRITTEN NOTE
Print this form (with instructions) or these blank forms as many times as you need OR send your own hand-written cards. Clearly print the student's first and last name or on the outside of the envelope. Mail or deliver to Sunset High School by 2:30 p.m. on Thursday, May 14 (13840 NW Cornell Rd., Portland, OR 97229 | Attn: Apollograms).
OPTION #2: SUBMIT NOTE ONLINE
Fill out the online form again & again! Volunteers will print & deliver your special note to seniors. Online form will close at 11:59pm on Thursday, May 14, 2026.
